Exempt Application Process

 

If you are unsure if you qualify for an exemption, please use the flowchart.  If it is determined that your research does not qualify for an exemption, you will be required to resubmit an application via the Standard Application Process, which will delay processing. 

 

Required Documents

    • Exempt Application

    • Conflict of Interest Statement  (for all Investigators)
    • Current human subjects' training certificate (for all Investigators)
    • Recruitment Materials: (Examples: E-mail to participants, flyer, intended telephone script, etc.)
    • Consent Process: In order for Investigators to protect the rights and welfare of human subjects, participants must give their consent to be a part of the study. The type of documentation required to obtain consent largely depends on the type of research being conducted. See HSPP 300-312: Ethical Considerations and Exempt Research and/or call the HSPP office to understand the best consent process for your study.  
    • Letter of Cultural Evaluation: For investigators conducting research outside the United States, a letter of cultural evaluation must be provided from an expert on the local culture where the investigator intends to conduct the research. Read Guidelines for Cultural Evaluation.
    • Survey Instrument and/or Interview Questions (if applicable)
    • Grant Proposal (if applicable)
