Continuing Review
It is the responsibility of the PI to submit a Continuing Review Application 30 days prior to the expiration date. In accordance with federal regulation, if a protocol expires, ALL research related activities must cease immediately. There is absolutely no grace period.
Once a protocol expires, a Continuing Review Application may be considered if submitted within ten days of expiration. If no response is received, a new application must be submitted for review and approval in order for the research project to continue.
Required Documents:
- Continuing Review Application
- Clean copy of current Consent Forms (i.e., not signed or date stamped)
- Conflict of Interest Form (for all investigators)
- Current Training (for all investigators): Investigators who did not complete the CITI course at the time of the Initial Review must submit a CITI completion report with the Continuing Review Application. Investigators whose CITI training is passed two years must complete refresher training and submit a refresher completion report.
