Standard Application Process

Required Documents

    • Standard Application
    • Conflict of Interest Statement (for all Investigators)
    • Current human subjects' training certificate (for all Investigators)
    • Recruitment Materials: (Examples: E-mail to participants, flyer, intended telephone script, etc.)
    • Consent Process: In order for Investigators to protect the rights and welfare of human subjects, participants must give their consent to be a part of the study. The type of documentation required to obtain consent largely depends on the type of research being conducted. See HSPP 600-605: Consent Process and/or call the HSPP office to understand the best consent process for your study. 
    • Letter of Cultural Evaluation: For investigators conducting research outside the United States, a letter of cultural evaluation must be provided from an expert on the local culture where the investigator intends to conduct the research. Read Guidelines for Cultural Evaluation.
    • Survey Instrument, Interview Questions, and/or Focus Group Questions (if applicable)
    • Grant Proposal (if applicable)
